Directions: Please have all necessary information before filling out this online form, since you are unable to save your progress. All fields with ‘s are required. The $10 registration fee can be paid via credit card or by check.
Students with complete applications including payment and forms will be accepted on a first come/first serve basis. You have the option of uploading your consent forms at the end of this application or mailing them in to secure your son’s spot. If you are paying be check, you must mail in your check to complete your son’s registration. After both the payment and consent forms have been received, you will receive an email with your official acceptance and more details about this semester’s BATS.
